UCLA Club Sports

1. Sign up on Ideal-Logic

    a. Search for Tennis in the Clubs tab and register

    b. Complete all required forms and Concussion Training
    c. Upload your PPE* form during registration on Ideal Logic
    d. Pay your Admin/Registration Fee ($65) on Ideal Logic 

2. Pay Club Tennis Dues ($45 per quarter, $80 per year)

    a. This can be done on Ideal-Logic once the rest of the membership is completed.

*You can schedule a PPE at The Ashe Student Health Center in one of the following ways:

• By phone: (310) 825-4073
• In person: 1st floor of the UCLA Arthur Ashe Student Health Center.
• Online: UCLA Arthur Ashe Patient Portal

These appointments are available for full-time UCLA enrolled students only. The cost of a PPE is $20 that will be directly charged to the student's Bruin Bill. PPEs need to be completed on our UCLA Club Sports Medical Form (see attached) by a Licensed Physician (M.D., D.O.), Physician Assistant (P.A.), or Nurse Practitioner (N.P.). Athletes who have submitted a cleared PPE in the past to the Athletic Training Center do not need a new PPE for the year.
